This year-long program is designed to provide an opportunity for providers and clinic leadership to participate without the significant demand for out-of-clinic time that places a burden on clinics. The program supports up to 40 leaders per year, and will include virtual trainings, four follow-up webinars and 1:1 leadership coaching. This program utilizes a cohort model, so please register alongside other leaders in your CHC. Examples of ideal cohorts include: a “pod” of clinic staff, a department team, clinic managers, etc. The minimum cohort size is 4 people, and the maximum cohort size is 10 people. Each cohort must designate one leader who will be responsible for leading their cohort on the team capstone or developing an independent capstone related to two UDS measures and patient satisfaction improvement.
